We urge you to call during regular business hours with any non-urgent concerns or questions. However, if you have urgent needs after hours, you can call our number and you will be connected with a pediatric provider who can assist you. Please be aware that there may be a fee for this service.

Mental Health and Behavior Concerns
If you are concerned about your child’s behaviors or mental health, please call our office to schedule an appointment. We may ask you to complete and send us one of the following forms prior to that appointment.
Development
If you are concerned about your child’s development, please call our office to schedule an appointment for further evaluation.
- See CDC Milestones for information on expected developmental milestones by age. You can also download a Milestone Tracker app at this site to monitor your child’s development.
- While we encourage you to call us if you are concerned about your child’s development, you can also call the Connecticut Birth to Three Program with any developmental concerns.

Forms
Health Assessment Forms
A Health Assessment Form is provided at each physical. We do not keep copies of the forms on file. Please makes copies of the form provided at your child’s physical.
There is a $10 replacement fee for lost Health Assessment forms. We will complete other necessary forms for camps, boarding schools and colleges at no additional cost.

Vaccines
At Hartford Area Pediatrics, we firmly believe in the safety and efficacy of vaccines to prevent serious illness and death.
Vaccinating your child is the single most important intervention we perform as your child’s pediatrician. We therefore require that all of our patients be vaccinated. We strongly encourage all of our families to vaccinate based on the current American Academy of Pediatrics schedule.
In rare circumstances when desired by families we will separate vaccines. Please be advised that deviating from the recommended schedule puts your child at risk for serious illness and goes against our medical advice.
There may be a charge for additional vaccine visits.
